Welcome to Eau Claire Public Schools! We are excited to welcome you into our Beaver Family! Once you have moved to the area and have received your new address, we ask that you complete the required enrollment forms necessary to enroll your child(ren) at Eau Claire Public Schools. See the requirements outlined below.  Additionally all the forms are now available electronically (see the left hand column for the links). If you prefer to print the forms, see the links below.  After you have completed the forms, please make an appointment to drop them off at the District Office (269) 461-6947. If you will be transferring from another school district, please complete a Release of Student Records form and submit it to Eau Claire Public School District, this will allow us to request your child’s records.

You will need to complete the forms and then make an appointment with the District Office.
You will need to provide the following documentation:

Documentation Required for Enrollment:
The following documents are required in order to enroll a student into Eau Claire Public Schools:

1. Student’s Original Birth Certificate with Official Seal (Required within 30 days of enrolling and shall be submitted in person.)

2. Student’s Immunization Records (student must be up-to-date or have a waiver signed by the Health Department)

3. Proof of Michigan Residency: Parent’s Driver’s License or State ID

4. 2 Required Proofs of Residency. This requires two pieces of documentation that show your current address.  See the table below.

Or, if you as the parents/guardians of the student are living with another family already residing in the district,  you may submit a notarized letter (or affidavit of residency) stating that you live with said person.

5. FOR KINDERGARTEN ONLY! The State of Michigan mandates that a vision screening be completed before the child’s first day of Kindergarten. Screening statements from the local health department, a licensed eye care practitioner or medical physician are acceptable.

6. If applicable, your child’s Individual Education Plan (IEP).

7. FOR HIGH SCHOOL ONLY!All enrolling high school students need to provide his or her most recent report card or High School Transcripts.
